    FK-506 is a novel macrolide with similar, but more potent, immunosuppressant activity than cyclosporine. FK-506 is used for primary or rescue immunosuppression in patients after solid organ transplantation. Clinically relevant side effects of FK-506 administration are nephrotoxicity, neurotoxicity, diabetes mellitus, and hypertension. Most of these side effects are related to blood levels of FK-506 rather than dosage. Therefore, therapeutic drug monitoring of FK-506 is a prerequisite for therapy. The whole blood target level range aimed for in clinical practice is 3.0-20.0 ng/mL and depends on the patient's liver function, the nature of organ transplantation, and the time after transplantation.
